The care navigator helps both the CKCC care management team and the nephrologist by providing care coordination to support patients / beneficiaries receiving treatment for chronic kidney disease. The navigator is a culturally competent health care worker who collaborates with patients, families, physicians, and the healthcare system to ensure chronic kidney disease patients' needs are appropriately and effectively addressed. A primary function of the navigator is to establish and help maintain communication between patients, their families, physicians, and the health care system, as well as to increase a patient's engagement in their own care and adherence to their treatment regimen. The individual who holds this position exemplifies the CVS Health mission in accordance with CVS Health policies and procedures, including complying with all CVS Health Customer Service Standards.Required Qualifications- 3+ years of experience working with CKD (Chronic Kidney Disease), ESRD (End Stage Renal Disease) or patients with multiple chronic medical conditions- 3+ years of experience in an ambulatory setting- 3+ years of experience with basic Microsoft Office programs required- 50% travel required within the Chicago marketCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated. Please note that in some states and roles, you may be required to provide proof of full vaccination or an approved reasonable accommodation before you can begin to actively work.Preferred Qualifications Maintain communication with patients, families, dialysis units, and the nephrologists to monitor patient satisfaction with chronic kidney disease care experience. Cultivate interpersonal relationships and foster trust with patients from chronic kidney disease (CKD) through end state renal disease (ESRD). Verify patient appointments and follow-up with nephrologists to assure planned dialysis start. Provide outreach for all program patients and coordinate care and services including access management throughout the lifecycle. Provide support and education for patients related to fluid management and sodium restrictions. Daily monitoring of inpatient discharges, ER discharges, and referrals. Monitor patient engagement in scheduled appointments for dialysis treatments, procedures, tests and consults. Work with care team to identify extrinsic barriers to adherence (i.e., transportation, financial concerns or pharmacy) and work with patient and team to alleviate barriers. Coordinate by phone with interdisciplinary care team to facilitate care coordination and ensure all patient medical needs are met. Candidate must reside in the Chicago area and have the ability to travel locally at least 50% of the year.
