Job DescriptionThe Kidney Care Social Worker will work remotely in the Chicago metro area. The Medical Social Worker (MSW) is a key member of the Care Management team in the Value-Based Care Operations Team. The Licensed Social Worker is responsible for supporting the social and emotional wellbeing of kidney patients. The MSW will conduct psychosocial assessments, educate patients, and conduct care planning to assist them in coping with issues (including emotional support, work, financial support, transportation and travel) associated with chronic kidney disease, end-stage kidney disease, dialysis treatments, and transplantation. The MSW will also provide psychosocial support to patients and their families to help them manage acute, chronic and terminal illnesses, including patient counseling and education, and referrals to other specialists and services. The social worker will work with the care management team to slow the progression of CKD, and also address barriers to optimal transitions, including home dialysis and transplantation. This is a market-based position. The MSW will work with patients and the care team through in-person, virtual, telephonic and electronic means of communications, care coordination and management. As a member of the care management team, the Licensed Social Worker will work with field-based clinical staff and physicians, non-clinical staff, and collaborate closely with transplant medical centers.Required Qualifications1.) Current license as a Social Worker in Illinois LSW or LCSW2.) Resides in Illinois 3.) Minimum of three years of previous experience in patient education4.) Minimum of 5 years of Renal social work experience5.)
