CVS Health is seeking experienced Family Nurse Practitioners to join our National Virtual Primary Care team. This position will report to the clinical care team lead.We are hiring 2 Providers for the Central Region. These full time, clinical positions are a work from home opportunity.The Central Region includes the following States: ND / SD/ NE / KS / OK / TX / MN / IN / MO / LA / WI / IL / KY / TN / MS / AL / ARYou will work in collaboration with a dedicated care team consisting of a Physician, RN and LVN to provide evidenced based care inclusive of accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning. You will perform all history and physical assessments of patients and evaluate and treat the patients' medical needs. You are responsible for all documentation within an electronic health record. You will collaborate with the care team and cross functional teams to ensure the delivery of high-quality care to the patient. As part of the CVS Health Virtual Primary Care team the FNP will be required to participate in professional development through organizational and departmental meetings, educational opportunities as well as participation on select committees as necessary.Required QualificationsActive Advance Practice Registered Nurse, Nurse Practitioner license in good standing.Current National Board Certification, Family Practice A minimum of 5 years of recent Primary Care experience as a Nurse Practitioner in either a family medicine or primary care practiceVirtual Care Team members must meet minimum requirements for remote care delivery, including broadband connectivity, a quiet setting with a neutral background to conduct visits from, and the ability to uphold patient privacy per CVSH guidelines. A minimum of high speed/broadband internet connectivity with a download speed of at least 25 download and 3 upload speed Must hold an unrestricted license in one of the states listed and have the ability to obtain multi state/ compact privileges and licensure in noncompact states as required by the business.COVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
