Job DescriptionAs a Clinical Trials Coordinator you are responsible for coordinating admission of patients from centralized benefit verification and intake to services provided by local pharmacy and nursing team.- Communicating directly with patient and/or caregiver to complete outstanding non-clinical patient information and Coram admission plan, and documenting and tracking outstanding forms for new admissions and therapy changes.- Communicates directly with patient/caregiver.- Communicates with centralized admission team, local pharmacy and nursing teams.- Documents non-clinical data, including patient demographics and documentation required on admission and therapy changes.- Identifies nursing level and schedules initial Coram or subcontracted home health nursing visit.- Performs other duties and responsibilities as assigned.Coram CVS/specialty infusion services is a Fortune 5 company and national leader in the home infusion and enteral fields.
