Health Coach Dietitian - Fully Remote

Job DescriptionThis is a full-time remote role with willingness for flexible hours desired. Standard business hours Monday- Friday with 1-2 late shift rotations per week potentially until 8pm CST. Saturday rotation required approximately once per quarter 8am-1pm CST. The Health Coach Dietitian works telephonically or by video platform with members to promote/influence members in decisions related to achieving and maintaining optimal health status. The goal of this program is to help members achieve healthy lifestyle behaviors and align these lifestyle behaviors with individual wellness goals. - Conduct telephonic/video and online clinical coaching assessments and sessions.- Provides recommendations for healthy behavior changes according to established protocols.- Assist members in weight management, quitting tobacco, stress management, nutrition, physical activity and other healthy lifestyle behavior change programs. - Identify co-morbid conditions, hospitalizations, prescriptions, and other factors that could impact the coaching and support needs of the member outside the scope of the health coaching program. - Support clinical integration and collaboration on member's care plan in conjunction with other health professionals.Required Qualifications- 3+ years of experience practicing as a Registered Dietitian- 3+ years of experience with healthy lifestyle coaching in at least 4 of the following areas: weight management, tobacco cessation, nutrition, physical activity and/or stress management. - 3+ years of experience with Microsoft Word, Outlook, PowerPoint, and documentation systems- Willing and able to obtain additional state licensures upon hireCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated. Please note that in some states and roles, you may be required to provide proof of full vaccination or an approved reasonable accommodation before you can begin to actively work.Preferred Qualifications- Certified Diabetes Educator (CDE) a plus- Certified Health and Wellness Coach- Other certificates or certifications related to health education/promoting behavior change- 3+ years of experience in facilitating patient/client positive behavioral change. - Experience in motivational interviewing and active listening- Experience with the Transtheoretical Model of Change, mindfulness-based and other evidence-based coaching techniques preferred.- Strong clinical decision-making skills.- Strong desires to coach, motivate, and support members.- Ability to work with a multidisciplinary team.- Experience or knowledge of management of chronic health conditions, especially as it relates to Diabetes, Obesity and Hyperlipidemia.- Demonstrated ability to communicate, problem solve, and work effectively with people, use good judgment in new and challenging situationsEducation- Bachelor's degree in a health related field required - Masters degree preferredBusiness OverviewBring your heart to CVS HealthEvery one of us at CVS Health shares a single, clear purpose: Bringing our heart to every moment of your health.
