Job DescriptionThe Transformation Consultant role is 100% remote, work from home and the employee can live in any state.This is a salaried position with an expectation to work Monday through Friday 8:00am-5:00pm in the time zone of residence with the ability to occasionally flex hours to attend meetings during CST time zone hours. The Transformation Consultant reports directly to the Director of Provider Transformation. The focus of the work is with project management and the employee does not need to have a clinical background.The Transformation Consultant is responsible for the following: -Assist with the design of the enterprise wide and/or cross-functional solutions to address Accountable Care Organization (ACO) business needs. This work results in the implementation of initiatives that impact ACO provider strategy enabling the Value Based Care (VBC)/Network teams to be competitive in the marketplace and effectively serve its customers. -Partners with business owners to optimize the achievement of strategic business objectives, to include profitable growth and improved organizational effectiveness. -Focus includes one or more of these areas: Management consulting, Business process/project consulting, Financial strategic analysis, Population health management, Physician governance and incentive models, Patient engagement models-Responsible and accountable for day to day tactical follow through of the transformation road map including implementation and tracking of programs within the ACO that will improve total medical cost, membership growth and achieve the Triple Aim.The work involves the following activities (not all encompassing):-Anticipates implementation challenges and develops solutions to overcome change resistance from Client thereby mitigating delays or challenges in attaining roadmap implementation objectives and timelines.-Develops transformational projects to position assigned clients for sustainable population health effectiveness across their full continuum of care delivery providers.-Supports between 8-10 ACOs. -Involved in initiative planning, ongoing client relationship management and customer satisfaction for assigned initiatives that impact the delivery of products and services to the internal and external environment. -Collaborates with external vendors, external consultants and Aetna team members as required in support of initiatives.-Accountable for transferring knowledge, best practices (external and internal), methodology and tools to client organizations. -Participates as a team member, may be responsible for concurrent initiatives or act as team lead for less complex projects based on resource requirements.-Utilizes project management principles and processes to promote successful population health management outcomes.-Provides support to assigned clinical transformation program implementation efforts.-Provides supports in coordinating project activities, meetings, and status reporting.-Collaborates with Informatics team to make sure all required reporting is provided in a timely manner and per scheduled client meetings.-Supports holistic solutions and/or key strategic plans. -Proactively identifies opportunities for improvement within assigned initiatives. -Participates and supports metrics integration and execution for optimal results. Shares joint accountability for initiative's return on investment.-Communicates compelling and concise results to empower client groups in the decision-making process regarding project opportunities. -Designs, develops interventions, and facilitates work sessions. Develops and delivers all required analysis, reporting and presentation materials necessary to communicate. -Gathers, analyzes, and synthesizes business intelligence to drive achievement of strategic business objectives -Builds and utilizes diverse internal and external networks, as appropriate, for data gathering and best practice identification. -Leverages internal and external networking to drive optimal initiative results and transfer knowledge and best practices.Required Qualifications-3+ years of project management experience in either a health care provider or payor environment. -3+ years of experience project managing moderate scale initiatives with moderate degree of complexity.COVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated. Please note that in some states and roles, you may be required to provide proof of full vaccination or an approved reasonable accommodation before you can begin to actively work.Preferred Qualifications-1+ years of experience working with Accountable Care Organizations-3+ years of experience successfully implementing change in complex organizations.-3+ years of experience with relationship management skills at the senior level; capacity to quickly build and maintain credible relationships at varying levels of the organization simultaneously.-3+ years of operational experience in a relevant discipline.
