Job DescriptionResponsible for determining optimal staffing coverage through a territory management plan utilizing a flexible staffing ratio of full-time, per-diem and agency nurses to maximize productivity that is consistent with the management and organizational goals.- Performs timely data entry of nursing notes in to the EMR. Tracks visit correction from nursing variance reports as directed Supervisor. Submits nurse visits for billing- Identifies nursing provider for new referrals, loads information into the EMR, provides orders and other communication to the nursing provider, and schedules known visits in the EMR. - Updates patient clinical and nursing demographic information in the EMR as needed.- Audits, tracks and obtains prescriber signatures on Plans of Treatment in collaboration with Primary nurses and Nurse Managers.- Noifies Coram nursing of therapy and order changes.- Manages master schedule for nurses working within ther assigned territories and adds to weekly nurse visit schedule for new and existing patients. - Completes Nursing Service Requests and Agreements as needed for collaboration between CVS Pharmacy and Coram nursing as well as between Coram Pharmacy and Agency nursing.- Retrieves submitted HHA invoices, verifies accuracy and submits to AP for processing.
