Health Coach Dietitian

Job DescriptionCandidates in MST, PST and CST highly preferred. This role is full time telework. Schedule is M-F, standard business hours required with flexibility needed for member calls.Position SummaryUtilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote/influence members in decisions related to achieving and maintaining optimal nutritional status.Assessment of Members:- Through the use of clinical tools and information/data review, uses knowledge and experience of nutrition and diet planning to conduct comprehensive evaluation of member's nutritional status and risks.- Utilizes assessment techniques to determine member's level of health literacy, technology capabilities/access, and readiness to change.Enhancement of Medical Appropriateness and Quality of Care:-Assimilate health/nutritional assessment data to establish a recommended nutritional approach based on member's health status/risks, education, preferences, and support needs- Partners with members to identify opportunities for integration into available internal/external programs. - Using holistic approach consults with supervisors or others to overcome barriers with assisting members in meeting goals and objectives.- Identifies and escalates quality of care issues through established channels.- Utilizes influencing/motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to achieve optimum level of health.-Utilizing interviewing and active listening skills, provides nutritional coaching, information and support to empower the member to make ongoing independent healthy lifestyle choices.- Interprets and utilizes clinical guidelines/criteria to positively impact members health.-Provides up-to-date information to help facilitate the member's understanding of his/her health status.-Helps member actively and knowledgably participate with their provider in healthcare decision-making.Required Qualifications- Registered Dietitian licensure through the Commission on Dietetic Registration OR must have equivalent licensure to a Registered Dietician in your state if you reside in AZ, CA, CO, HI, IN, MI, PR, VA, & Virgin islands- Upon hire, must be willing and able to obtain additional licensures in multiple states where members reside, with CVS supporting the cost of required licensure.- Certified Diabetes Care and Education Specialist (CDCES) required. - 3+ years of experience working in a multidisciplinary team required. - 5+ years of experience with weight management, disease management, medical nutrition therapy or health lifestyle behavior change required. - 3+ years of coaching experience required.COVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
