Job DescriptionThis is a remote, work from home role, and can be located anywhere in the US. This Clinical Pharmacist - Medicare is responsible for member-facing clinical telephonic outreach and market engagement to support Medicare Part D STARS initiatives. The Clinical Pharmacist in this position will coordinate, plan, and implement quality initiatives for the clinical pharmacy team supporting Medicare STARS.Responsibilities of this position include: Oversee and execute clinical pharmacy programs Coordinate strategies for increasing adherence Utilize motivational interviewing and health coaching to impactmember behavior Ensure effective collaboration with internal and external matrix partners to support clinical programs Work collaboratively with leadership and management to strengthenbusiness and operational processes Provide clinical pharmacy support to programs targeted toward the Medicare membership Other duties as requiredThis role requires established experience in functions beyond fundamental dispensing or order-procesing activities, or prior authorization approval/drug utilization review. This position does involve potential travel for meetings and member events, up to 10%.Pay RangeThe typical pay range for this role is:Minimum: 82000Maximum: 176800Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications- Current unrestricted state pharmacist license- Doctor of Pharmacy degree from an accredited school of pharmacy- 3+ years experience working at a health plan managing Medicare STARS to improve quality outcomes for members. - Established experience in functions beyond prior authorization approval or drug utilization review required. OR 3+ years in a Clinical Pharmacist Role which must include at least one of the following:o Rounding and making therapy recommendations as an active part of a multi-disciplinary care teamo Independently managing patient therapy under a collaborative practice agreemento One year of a Pharmacy Practice ResidencyClinical Pharmacist Role: Established experience in functions beyond fundamental dispensing and order-processing activities such as optimization of medication regimen and monitoring or pharmacist-led care management Experience with population health and/or disease state management Ability to research and analyze medication literature including drug information, disease states, and clinical practice guidelines to identify gaps in care and create/communicate a recommendation plan to providers and patientsCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
