Job DescriptionInvolved in initiative planning, ongoing client relationship management and satisfaction for assigned initiatives that impact the delivery of products and services to the internal and external environmentCollaborates with external vendors, external consultants, consulting team members and/or matrixed staff as required in support of initiatives.Accountable for transferring knowledge, best practices (external and internal), methodology and tools to client organizations. Participates as a team member, may be responsible for concurrent initiatives or act as team lead for less complex projects based on resource requirements.Supports holistic solutions and/or key strategic plans. Proactively identifies opportunities for improvement within assigned initiativesImplement and scale the overall strategy to address SDoH for members and communities across stateOperationalize SDoH product in local markets Hire, train, and manage field staff in assigned markets Establish and build key relationships with community-based organizations to execute SDoH strategyIdentify opportunities to develop and pilot new SDoH initiativesGather input and maintain proactive communication across business units to coordinate SDoH function across the marketPay RangeThe typical pay range for this role is:Minimum: 60300Maximum: 126600Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications Bachelor's or higher in health administration, social services, public health, business administration, psychology or closely related field Licensure or certification if applicable to degree 5 + years' experience with population health, care coordination, and/or community-based programs 3 + years in a leadership role Experience leading in a matrix environment Excellent interpersonal and strong communication skills Demonstrates good oral and written presentation skills Strong organizational skills, including the ability to prioritize, multi-task, and manage workload to meet specific timeframes and deadlines Prior Managed Care ExperienceCOVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
