Responsible for establishing, implementing and evaluating a plan of care to restore function and prevent disability following disease, injury or loss of a body part.Responsibilities • Performs thorough evaluation • Develops appropriate Plan of Care • Implements Plan of Care • Documentation is accurate, complete and timely • Provides supervision to assistants, rehab techs, and students Qualifications • Masters Degree in Physical Therapy or Bachelor''s degree in Physical Therapy with a minimum 5 years of work experience. • Illinois license in Physical Therapy or eligible • CPR PIQrehabPandoLogic.
