Job DescriptionThis is a remote position with expected travel up to 25-50% of the time for member facing visits. The schedule is Monday-Friday, standard business hours, 8:00am-5:00pm CST. Candidates must be located in Collin County, Texas or an adjacent county. The care manager is responsible to coordinate and collaborate with member/Legal Authorized Representative (LAR), PCP, and any other care team participants to ensure member's Long-term Service and Supports (LTSS) are optimized. The care manager will attend interdisciplinary meetings and advocate on members behalf. They will work with member/LAR to complete state's Screening and Assessment Instrument (SAI) and works with care team to develop care plan and will authorize services within the LTSS benefit. The care manager will also work with the member/LAR to coordinate with other care team participants, facilitate and assist member regarding assessing community resources to support any unmet member needs. They are responsible for documenting accurately and timely in the clinical documentation system platform. This position requires the care manager to use critical thinking and be able to problem solve any issues related to assigned membership. The BH Clinical Case Manager is responsible for conducting face to face visits using comprehensive assessments of members enrolled in Long-Term Services and Supports program (LTSS)Pay RangeThe typical pay range for this role is:Minimum: 55,300Maximum: 118,900Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls.
