Job DescriptionQualifying candidates are eligible for an up to $5,000 sign on bonus. Candidates holding a NJ Choice Certification are eligible for an up to $10,000 sign on bonus.Develop, implement, support, and promote healthservice strategies, tactics, policies, and programs thatdrive the delivery of quality healthcare to our members.Health service strategies, policies, and programs arecomprised of utilization management, qualitymanagement, network management, clinical coverageand policies. The position requires advanced clinicaljudgment and critical thinking skills in order to facilitateappropriate physical, behavioral health, psychosocialwrap around services. The care manager will beresponsible for, care planning, direct providercollaboration, and effective utilization of availableresources in a cost-effective manner. Strong assessment,writing and communication skills are required.Fundamental Components/Job Description:The Case Manager is responsible for conducting face toface visits in the members home utilizing comprehensiveassessment tools for members enrolled in ManagedLong-Term Services and Supports program (MLTSS).Successful completion of company sponsored NJ ChoiceCertification is requirement for continued employment.The case manager is responsible for coordinating andcollaborating care with the member/authorizedrepresentative, PCP, and any other care teamparticipants. The case manager schedules and attendsinterdisciplinary meetings and advocates on themembers behalf to ensure proper and safe dischargewith appropriate services in place. The case managerworks with the member and care team to develop a careplan and authorizes services in a cost-effective mannerwithin the MLTSS benefit. The care manager isresponsible for documenting accurately and timely in themember's electronic health record. This position requiresthe case manager to use critical thinking skills and theability to problem solve. The Case Manager will also beexpected to mentor new hires, once, a level ofproficiency has been attained in their role.Pay RangeThe typical pay range for this role is:Minimum: 19.52Maximum: 38.99Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications2+ years of paid professional experience working directly with the elderly or physically disabled in an institutional or community setting.- Willing and able to travel 50-75% of their time using your own vehicle to meet members face to face in Middlesex County NJ and surrounding areas/counties- Valid driver's license required- Successful completion of the NJ Choice certification is a contingency of employment.
