The Opportunity Perform post-discharge follow-up calls regarding a variety of subjects that are not limited to discussing results of studies, clarification of discharge instructions, and answering patient care questions. Adheres to appropriate and complete documentation practices, including the history of present illness, adjustment or maintenance of an established treatment plan, and consistent follow-up of the plan as evidenced in the documentation. Document all contact, communications in patients'' electronic medical record. Communicate with medical providers, the employer, and other stakeholders as required. Maintains accurate detailed reports, tracking log of callbacks with details as requested by MD, and records and directs care with regards to age and developmental stage, presenting complaint, and pertinent history. Initiate and follow through on patient callback process, identifying opportunities for service recovery or continued care.
