Job DescriptionThe Senior Manager of Population Health will be responsible for strategizing and leading data organization, structure, and analysis to support actionable insights resulting in improved access to care, improved outcomes, and cost savings. Working under the direction of the Director of Quality and Population Health, this position identifies, develops, and implements strategic NCQA-based Population Health programs that support Aetna Better Health/CVS vision and short-and-long term goals specific to health outcomes of members with chronic diseases and disadvantaged populations of focus. This role is critical in supporting internal and external health equity efforts to reduce disparities.This role will work within a context of a multidisciplinary team of both internal and external stakeholders, identify opportunities to enhance member outcomes through collaborative initiatives that address gaps in care, and strengthen Plan relations with external stakeholders at both the public health and community levels to advance health equity initiatives for the organization. These partnerships with external stakeholders include federal, state, and local levels, and are necessary to advance the organization's interests, and ongoing contracted, provider and vendor delivered programs with a focus on quality, accessibility, sustainability, and integration within Aetna Better Health.- Previous experience in project management, healthcare economics, and data analysis Ability to research and analyze information, problems, situation and procedures to define objectives, identify patterns, and formulate conclusions Excellent problem resolution abilities Solid supervisory background, including selection, training, delegating, motivating, performance appraisal, career counseling and discipline Experience in clinical planning, financial analysis and business planning Background in a major research-oriented school Track metrics and develop targeted solutions to increase member engagement. Leads implementation of population health activities related to pilot programs. Skilled in analyzing gaps in care related to HEDIS measures and integrating into population health programs and initiativesAbility to work in a matrix environment knowledgeable in incorporating population health metrics in designing value based programsVery strong understanding of clinical programs and pop health initiatives such as HEDIS, STARS, P4PProven track record of clinical program rollouts for small and large populationsExtensive experience with various member and population health management softwareAdvanced MS Office, various reporting tools, and presentation skillsExcellent written and verbal communication skillsA strong work ethic and an ability to work well in a team-based environment.Leads key strategic and transformative initiatives around Population Health and Health EquityPartners with clinical and business leadership to drive cost of care utilization management by identifying disparity populations and opportunities; assists with designing action plans and developing /tracking key performance indicators- Facilitate efforts with all levels of the organization - clinical, financial, administrative, and information systems - that employ member data in decision-making and everyday operations by improving clinical and analytical reporting.- Create and develop processes to address health disparities- Activities include identifying disparity populations, designing strategic interventions, and establishing key measurement indicators.Pay RangeThe typical pay range for this role is:Minimum: 75,400Maximum: 158,300Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls.
