Join us in our efforts to better our community!What you will do in this role:You will be responsible and accountable for coordination of services through an interdisciplinary process which provides a clinical and psychosocial approach through the continuum of care and balances individually identified patient and family needs with the efficacious and cost effective utilization of resources. Through concurrent case management, you will assess patients to determine appropriateness of admission, continued hospitalization, as well as appropriate level of care. You will conduct discharge planning that will begin at the time of (or prior to) admission, and reassessed ongoing throughout the course of hospitalization. You will monitor Quality and Risk Management issues for and report to the QRM department as a part of the CQI process.Swedish Medical Center, part of HCAs HealthONE, is located in the south metro Denver area where it has been a proud member of the community for more than 110 years. What qualifications your will need:Current licensure in the State of Colorado as a Registered Nurse, or current active multistate nursing licensure. BLS certification through AHA or ARC.Graduate of an accredited school of nursing; BSN preferred.3-5 years recent clinical or case management experience in area of specialization.Requires excellent human relations and oral/written communication skills. Must have the ability to asses information, apply critical thinking processes to determine course of action, define expected outcomes, and evaluate effectiveness of plan through continuous assessment of the patient''s progress.Must have the ability to compile, evaluate and report statistics to members of the team, and utilize the information to facilitate process improvement activities. Strong communication skills are necessary for meeting facilitation, educational presentations, team collaboration and patient interviews.
