Were looking for Nurse Practitioners (NP), Physician Assistants (PAs), Licensed Social Workers (LCSW), and Community Health Workers (CHW) who believe serving seniors IS their calling, their reason for jumping out of bed every day. The role Working in small teams (Care Pods) with other highly capable and passionate clinicians, you will Develop high-quality therapeutic alliances with patients and their caregiversGuide patients and their families through the journey of dementia and late life mental illness Provide ongoing cognitive, behavioral and psychopharmacological assessments and treatments Coordinate patient care with primary care providers and specialists, nurses and home health aides Coordinate patient care with Rippl social workers and community health workersProvide advanced care planning for both patients and families Provide dementia care planning and chronic care management What youll bringA deep passion for taking care of older patients and developing longitudinal care relationships with them and their families Interest in learning a new skill set and pioneering a new workforceComfort working with interdisciplinary teamsAn unrestricted, current WA State license to work as an advanced practice registered nurse or physician assistant (board certified or board eligible); Mental Health, Geriatric Medicine, Palliative Care or Chronic Care management experience preferred1-2+ years of experience managing older adult patients in outpatient, inpatient or long-term care settings (preferred but not required) Interest or proficiency in psychotherapy and psychopharmacologyValid driver''s license and proof of motor vehicle insuranceWhats in it for you Our most important responsibility as a company is to ensure youre supported in every way possible to be ready every day to serve our patients.
