Unlock your potential!Job Summary and QualificationsThe RN Case Manager will facilitate the interdisciplinary plan of care with a focus on evaluating the appropriateness of clinical care, medical necessity, admission status, level of care, and resource management. Responsible for coordinating activities that promote quality outcomes and patient throughput while supporting a balance of optimal care and appropriate resource utilization. Identify potential barriers to patient throughput and quality outcomes and will facilitate appropriate discharge plans.Schedule: PRN / Per Diem / As Needed What qualifications you will need:Associate Degree in Nursing or diploma in Nursing requiredRegistered Nurse (RN) license requiredCertified Case Manager preferredMinimum of 1 year of Case Management experience preferredThis role requires you to be fully vaccinated for COVID-19 based on local, state and /or federal law or regulations (unless a medical or religious exemption is approved).HCA Florida West Hospital is a 515-bed facility.
