Health Home Care ManagerPlainview, NYThe role of the All About Kids Care Manager is to manage, coordinate, and link other services as needed in the community through Health homes and Early Intervention to children and families dealing with emotional, physical, and/or developmental delays. The best candidates for this position will have an excellent track record of at least 2 years of case management experience, clear verbal and written communication, as well as outstanding organizational and time management skills.This position is currently being conducted remotely, however the ideal candidate will be capable of traveling to our Plainview headquarters to retrieve company issued equipment to start, and potentially work in the office on very rare occasions when it may be necessary.ResponsibilitiesSupport families, doctors, and therapists, act as an intermediary between Care providers and familiesEnsure that Health Home and /or Early Intervention Services are provided in accordance with NY State and Early Intervention Program and Health homes rules and regulations; monitor the frequency and duration of the Individualized Family Service Plan (IFSP)Coordinate with therapists, parents, and Service Coordinators to facilitate timely and consistent provision of servicesCommunicate with pediatricians and medical personnel to obtain prescriptions and medical clearance when neededFollow up with therapists to request and obtain progress reports in a timely manner when neededConduct quarterly parent surveys to ensure parents satisfaction with servicesDocument dates of contact, progress reports, first attend dates, frequent or consecutive absences, and any other important information pertaining to the childs Early Intervention services or Health Home services in the case activity logUpdate the companys database and ensure it contains accurate informationMaintain positive professional relationship with Service Coordinators and service providersRespond to e-mail and phone inquiries from Service Coordinators, therapists, and office staff promptlyCollaborate and communicate with fellow office staff members across all office locations effectively - both in-person and via email, telephoneTravel between office locations once per month or as neededTravel for in-person meetings with familiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job. Duties, responsibilities and activities may change at any time with or without notice.Required Education and Experience2 years related job experienceStrong computer skills; proven experience with full MS Office Suite and Case Management database(s)Clear verbal and written communication skillsMust be organized and efficient in managing daily routines and workflowMust be able to problem solve, meet deadlinesMuch complete CANS-NY training upon 4 weeks of hire or have prior CANS-NY certificationBachelors degree in related field of study, BSW or MSW preferred.New graduates welcome.
