Job Description Candidate must reside in PA - Develop and implement strategies and policies to direct our operations, subsidiaries, major projects and functional groups. - Help our business units - and our company - meet corporate objectives and compete more successfully in the marketplace. - This work results in initiatives that significantly impact company strategy enabling Aetna to be competitive in the marketplace and effectively serve its customers. - Partners with business owners to optimize the achievement of strategic business objectives, to include profitable growth and improved organizational effectiveness. - Interacts with external vendors, external consultants, consulting team members and/or matrixed staff as required in development of initiatives. - Develops and supports transferring knowledge, best practices (external and internal), methodology and tools.- Participates as a team member, may develop concurrent initiatives based on resource requirements. - Implement and scale the overall strategy to address SDoH for members and communities in assigned region- Operationalize SDoH product in assigned region- Learn and understand available community resources and programs - Utilize data and local knowledge to build grassroots, strategic and data informed community partnerships- Identify opportunities to develop and pilot new SDoH initiatives- Establish outcomes-based contracts with community-based organizations- Organize community activation meetings to successfully execute key SDoH initiatives with community partners - Cultivate relationships with internal and external stakeholders to drive collective member impactPay RangeThe typical pay range for this role is:Minimum: 43,700Maximum: 97,400Please keep in mind that this range represents the pay range for all positions in the job grade within which this position falls. The actual salary offer will take into account a wide range of factors, including location.Required Qualifications- Must live in Pennsylvania - 3 + years' experience with population health, care coordination, and/or community-based programs- Ability to travel up to 50% of the time within the assigned region, to multiple areas with personal vehicle.- Must possess reliable transportation, valid/active driver's license, and proof of automobile insurance.COVID RequirementsCOVID-19 Vaccination RequirementCVS Health requires certain colleagues to be fully vaccinated against COVID-19 (including any booster shots if required), where allowable under the law, unless they are approved for a reasonable accommodation based on disability, medical condition, religious belief, or other legally recognized reasons that prevents them from being vaccinated.
