Newport House is seeking a Social Services team member for On-Call shifts. This position could develop into additional structured hours in the future.The overall purpose of this job is to advocate for residents and to assist in meeting psychosocial, social, rehabilitation, financial and discharge planning needs of residents, families and responsible parties. Assists with the social service programs in accordance with current existing federal, state, and local standards to ensure medically related emotional and social needs of the residents are met. This position may also be responsible for marketing for potential residents and families seeking care. Supports and lives out Immanuels Mission and CHRIST Promises.Key AreasKey Responsibilities and Duties of the JobResident CareAssess needs of residents and maintain care plete and document required assessments and discharge planning as needed.Coordinate and assist admissions to community.Develops a timely and effective discharge plan with interdisciplinary team. Documents assessment and discharge planning within the medical record.Responds and intervenes in the resolution of crisis or conflict situations. Provides resources and support to residents and responsible parties surrounding the issues of illness, loss, grief and bereavement. Documentation in the medical record, consistent with and limited to the scope of the job duties and any applicable licensure. Ensures 100% of resident population is screened for psychosocial needs within 72 hours of admission and develops plan for physician and health care team.DutiesDevelop and maintain comprehensive resource files to empower residents and responsible parties the ability to make choices about needed services and resources. Assist residents and responsible parties in applying for government assistance programs as needed.Assist and educate on legal documents such as Advance Directive.Attend weekly care conferences with residents and families.Market community services as needed.Maintains updated documentation and timely MDS completion in LTC.Collaborates with the health care team and serves as a liaison with community and referring agencies by developing and maintaining good working relationships.Must have ability to work effectively and collaboratively in a diverse and multi-cultural environment. Assist with the investigation of allegations of abuse/neglect and report to APS/State agency if required. Educate as required. Educate residents, responsible parties and team members about resident rights. Manage the grievance process and documentation. OtherPerforms other duties as assigned or requested.Education- Bachelors Degree from an accredited school in social work, social services field or gerontology.Equivalent years of experience may substitute for education requirement.
