POSITION SUMMARY:Review, evaluate, and dispense individual drug orders including unit dose medications, intravenous additive preparations, total parenteral nutrition, chemotherapeutic agents, and investigational agents; interact with nurses and prescribers to consult on medication and treatments along with drug therapy; monitors the drug therapy and offers cost-effective alternatives, as appropriate; counsels patients in the proper use of prescribed drugs; oversees and directs pharmacy technician staff in the execution of technical functions.ONCOLOGY-SPECIFIC DUTIES:Oncology pharmacists are actively engaged in all aspects of cancer care.Perform chemotherapy dose preparation and safety checks.Perform daily evaluations of medication profiles to ensure each drug is dosed appropriately.Work with the medical and nursing staff to maximize the benefits of drug therapy while trying to minimize toxicities.Work with medical and nursing to assist with patient''s supportive care plan, which often focuses on pain, neuropathy, nausea, or vomiting.Educating patients about side effects and worrisome drug interactions on their current med list. Council patient or caregiver as it pertains to any questions about a cancer medication upon request.Help coordinate the complete medication plan, from inpatient chemotherapy infusions to what medications need to be taken at home.Perform drug development research and educate staff on new medication deliveries.Maintain appropriate inventory control at Cancer Center. It is up to the pharmacy to drive fiscal responsibility and waste reduction.Work with the prior authorization team, medical staff, and nursing to assure that all medications are delivered appropriately and the regimens are payable.Point person for Cancer Center''s EHR system and Cancer Center''s Pyxis (ADMs) as it pertains to medication delivery.Directly manage all support staff within the Pharmacy location in the Cancer CenterFUNCTIONAL RESPONSIBILITIES:Must have extensive knowledge of drug protocols, medical therapeutics, pharmacokinetics, and disease states. Must have the ability to work with information systems, automated and technical equipment. Must possess word processing, database, and spreadsheet capabilities. Must possess good communication and interpersonal skills. Must have good organizational skills. Must have the ability to follow policies and procedures and individual instructions. Must have the ability to prioritize tasks.PRIMARY DUTIES:Provides specialized pharmaceutical services to improve drug usage and therapeutic outcomes including advising physicians on issues concerning drug therapy, the inherent toxicity of drugs, and side effects; assists in the prescription of appropriate doses.Coordinates the activities of assigned pharmacy technicians engaged in the preparation, labeling, and delivery of medications and other pharmaceutical supplies in accordance with prescribers'' prescriptions.Ensures compliance with federal, state, and local laws and regulations pertaining to the dispensing of drugs and controlled drugs and the maintenance of required records.Assumes the essential responsibilities and performs the duties of the pharmacy supervisor in his/her absence.Mentor pharmacists in training including, but not limited to, pharmacy students during their IPPE and APPE rotations and pharmacy residents.Maintains liaison relationships with medical and nursing staff, and provides timely information pertaining to pharmaceutical supplies, drug usage, compatibility, state and federal regulations regarding drug controls, and the Joint Commission standards.Assumes compliance accountability and responsibility for Oneida Healthcare policies and procedures, accreditations standards, and applicable federal, state, and local laws and regulations.Conducts and evaluates medication histories; assesses compliance and suggests modifications so as to achieve desired outcomes; instructs patients in the proper use of prescribed drugs; participates in patient care rounds with multidisciplinary teams to evaluate patient dividualizes medication regimens using sound principles, accounting for pharmacodynamic and pharmacokinetic variations in drug absorption, distribution, metabolism, and elimination with responsibility for establishing and continually improving the delivery of pharmaceutical care to patients within the areas of direct responsibility; assists others in the department with the same.Provides patient-focused clinical pharmaceutical care which includes, but is not restricted to, primary pharmacokinetic care and anticoagulation services.Conducts and participates in research including, but not limited to, performing as the primary investigator or co-investigator for research programs that will impact the delivery of quality care or examine the pharmaco-economic impact of providing care to the patients of Oneida Healthcare.Identifies adverse drug events (ADEs) and assists in their management and prevention; develops process improvements to reduce drug errors and preventable ADEs; participates in reporting ADEs to institutional committees and to the Food and Drug Administration''s MedWatch program, if necessary.Documents clinical activities that include, but are not limited to, disease-state management, general pharmacotherapeutic monitoring, pharmacokinetic monitoring, ADEs, education, and other patient care activities.Provides educational in-services to pharmacy staff and other healthcare professionals both formally and informally.Attends meetings as required, including pharmacy department meetings (including Journal Club), multidisciplinary meetings, service-specific meetings, and educational in-services.Provides primary oversight for specific operational areas assigned, which might include but are not restricted to, Quality Control, Nutrition Support Liaison, Training, and Competency Monitoring, Sterile Products Productions Standards, and Medication Safety.Maintains established departmental policies and procedures, objectives, quality assurance programs, safety, and compliance standards.Enhances professional growth and development by participating in educational programs, reading current literature, and participating in in-service meetings and workshops.Prepares written documentation as required by the profession and the department; maintains, distributes, analyzes, and projects information for required records, reports, and statistics as directed.OHHPQUALIFICATIONS:Education and Experience:Doctor of Pharmacy degree; orMaster''s degree and the equivalent of one (1) year of full-time clinical pharmacy or pharmacy-related experience; orBachelor''s degree and the equivalent of three (3) years of full-time clinical pharmacy or pharmacy-related experience.Licensure:License to practice as a Registered Pharmacist in the State of New York
