Salary Range: 76,960 - 106,080 Responsibilities VIPC Clinical Liaison Responsibilities Provide community and provider in-service education in assigned territory. Maintain daily communication with Admissions Department and Team Directors. Work with the Hospice Care Consultants to develop and implement individualized marketing plan. Maintain visibility and accountability to increase relationships with contracted inpatient facilities and physician groups. Act as a liaison with AccentCare Medical Director, Team Physicians and Hospice Physicians on behalf of providers and patients/families. Make inpatient visits at contracted facilities and communicates with Admissions Department, Team Directors and/or Primary Nurse. Provide consultation on hospice and palliative care. Demonstrate familiarity with policies of the agency and state(s) and federal regulations. Participate on committees and special projects and other related duties as assigned. Perform evaluations, sign ups, assessments, and discharge planning of new patients primarily in contracted hospitals. Formulate the initial interdisciplinary plan of care and communicates to the Team Director Obtain data on physical, psychological, social and spiritual factors that may influence patient/family health status and incorporate that data into the plan of care. Turn in all paperwork pertaining to sign ups and assessments within 24 hours to Clinical Services. Maintain confidentiality of sensitive information. Participate in hospice orientation and ongoing education programs. Attend outside seminars or educational opportunities to increase knowledge and skills. Performs other duties as assigned including those within the scope of current, valid professional licensure/credentials (i.e. RN, SW, Chaplain, MT, HA, etc.). Qualifications Clinical Liaison VIPC Qualifications: Registered Nurse Qualifications: Registered Nurse license in state(s) practicing. Board Certification or working toward board certification in hospice and palliative care. Previous experience in hospice, home health, general medical surgical or patient population specific (i.e.: geriatrics, pediatric, etc.) nursing. Social Worker Qualifications: Qualifications vary by state. Hospice will apply the requirements per the Medicare Conditions of Participation for all Social Workers hired after December 2, 2008, except in the state(s) which require a higher minimum level of training and/or licensing. Medicare Conditions of Participation (COP) Qualifications for Hospice Social Worker: Has a Master of Social Work (MSW) degree from a school of social work accredited by the Council on Social Work Education; or Has a baccalaureate degree in social work from an institution accredited by the Council on Social Work Education; or a baccalaureate degree in psychology, sociology, or other field related to social work and is supervised by an MSW as described in paragraph (b)(3)(i)(A) of this section, and Has one year of social work experience in a health care setting; or Has a baccalaureate degree from a school of social work accredited by the Council on Social Work Education, is employed by the hospice before December 2, 2008, and is not required to be supervised by an MSW. Qualifications for state-specific requirements above and beyond the Medicare COP Minimum one year of social work experience in a healthcare delivery system is required; two years experience is preferred. Must have reliable transportation, a valid drivers license, and the minimum state required liability auto insurance.
