RN - Registered Visit Nurse - Hospice - On Call

Description Are you seeking something with flexibility and away from the 8-5, M-F routine We are seeking RN''s to cover on call with per visit compensation and beeper fee. Additionally, per diem weeknight shifts, 5pm-8am, are open as we grow and have increasing coverage needs. Potential for more hours as we expand our organization. Patient care provided in the community, at home, or care facilities as appropriate. Responsibilities include, but not limited to: Must be willing to work every-other weekend. Weekends begin at 5pm on Friday, and end at 8am on Monday. Must be organized and self-directed in the field Visits as needed to assist RN Case Managers in covering their patients Participates in on-call coverage On call shifts with beeper fee and a per visit pay schedule Assesses and evaluates patients'' level of care, needs, and requirements Submits paperwork in a timely manner and updates and maintains the patient medical records Ensure our patients receive top-notch care Evaluating outcomes of care Collaborates with the physician and other team members as needed Applies specific criteria for admission and re-certification of hospice care to establish appropriate levels of care and the patient''s eligibility Attends deaths of patients to assist and support family Consults and educates the patient/family regarding the disease process, self-care techniques, end-of-life and the processes for dealing with issues of ethical concern Evaluates own needs for support and uses identified systems to meet the need Accepts other assignments deemed as appropriate In return for your expertise we offer generous compensation, mileage reimbursement, supportive knowledgeable team and the list goes on! Location 4657 - ProMedica Hospice - Serving Twin Cities, Minnesota Educational Requirements Currently licensed as a Registered Nurse in the state and in good standing with the Board in which he or she will practice. Position Requirements A minimum of 6 months institutional nursing experience within the past 3 years, unless state regulations differ. The DON or Administrator may waive this experience requirement Registered Nurses providing skilled intervention for PASSPORT Patients must have 2 years experience as an RN.
