Recognizing that health depends upon an individuals economic, educational and employment status, FHCs Department of Community-Based Programs offers an extensive continuum of family strengthening and community development programs to address the broader needs of the community, including adult education; case management and supportive services; mental health services; early childhood centers; and community service opportunities. The Sunset Park Family Child Care Network is a place-based initiative that provides support and monitoring to their affiliated family child care providers to ensure the delivery of high-quality services to children and families.The Family Health Centers seek to support and enhance the existing strengths of family child care as birth-3 programs that promote continuous and responsive relationships, strengthen early learning skills, build resilience, and lay the foundation for all future learning. The primary role of the Program Counselor is to oversee the provision of family and social support services for enrolled families and affiliated family childcare providers, as well as the ongoing outreach and enrollment of new affiliated providers and families. The Counselor will supervise the Network''s Family Workers and support building and sustaining effective relationships with families and affiliated providers to ensure the needs of all enrolled families and children are met. The Counselor will work as part of an integrated team of administrative and support staff.Job Responsibilities: Provide reflective supervision to the Networks Family Support Workers. Work with Family Support Workers, families and affiliated providers to create individualized family plans in the provision of family and social support services. Oversee ongoing outreach and enrollment of families and Family Childcare Providers Oversee the coordination and implementation of monthly family engagement workshops, special events, and other activities with the FCC Network team to encourage and maintain family participation. Ensure access to ongoing services for families/affiliated providers needing additional support services (special education, mental health services, housing support, etc.), connecting them to appropriate resources and making necessary referrals to community partners. Collaborate with Family Support Services leadership to align workflows, processes and procedures, and training for Family Workers. Develop relationships with community partners who provide mental health, special needs, and other support services for all affiliated providers and families. Oversee the enrollment process of families including recruitment, registration, intake, family assessment, and ongoing support to families. Support families of children with special needs by attending case conferences and follow-up meetings. Collaboratively create and update a Community Partner Resource Guide for all providers and families. Document all family conversations and sessions in a timely manner and align with the Early Childhood Framework for Quality (EFQ). Participate in regular staff meetings to connect about individual providers, families, and coordinate all services. Participate in Network staff professional learning sessions delivered by the Department of Education. Other tasks assigned by Program Leadership. Minimum Qualifications:To qualify you must have a LMSW or LCSWExperience working with families with young childrenBilingual Spanish. Required Licenses: Licensed Master Social Worker or Licensed Clinical Social Worker, Preferred Qualifications: Experience working in family child care and/or with infants and toddlers (preferred) Willingness to travel to providers homes in Brooklyn. Time management skills are essential. Ability to multitask. Must be a team player. Appreciation for the unique needs of all providers and families.
