UAS FIELD ASSESSMENT RNFFS TELEHEALTH POSITIONS AVAILABLE!AgeWell New York is seeking a UAS Assessment Nurses (RNs) to join its team. Conveniently located in Lake Success, NY, AgeWell New York is a managed care company that is rooted in the communities and culture of the New York metropolitan area.Understanding what it takes to support the senior population across the continuum of health care needs, AgeWell New York stands ready with dedicated professionals and expert staff, support resources, and comprehensive care coordination to assist members optimize health care coverage.We are seeking experienced, reliable and highly motivated Assessment Nurses to conduct in home assessments for prospective and current members to determine initial plan eligibility, continued eligibility and their health care needs using the UAS tool.Specific duties include, but are not limited to:Performing comprehensive in-home and telehealth history assessments of members to determine needs and eligibilityEnsuring proper documentation for all referrals and members and communicating effectively with Enrollment Staff regarding the acceptance of the referralEducating and communicating with members about their medical conditions and providing interpretation services as neededProviding education on disease prevention and management, and health promotionThis position requires a current New York State Nursing license and two years minimum experience in home care or care management. The ideal candidate must possess excellent listening and communication skills, the ability to complete documentation in a timely manner, as well as attention to detail and accuracy when performing assessments. Must have access to reliable transportation. Bilingual in Spanish or Mandarin/Cantonese highly preferred.
