Job Description

Homeland HomeHealth is looking for a compassionate and caring Registered Nurse, who is driven by a desire to provide the highest quality of care for our patients; has a strong attention for details; excellent problem-solving and communication skills; is independent and self-motivated; and is adept at prioritizing and balancing multiple tasks simultaneously. This is a salaried position that provides On-Call coverage from 5:00pm Friday through 8:00am Monday and is paid bi-weekly at 37.5 hours per week. Provides skilled nursing care to home health patients as prescribed by the physician and in compliance with the Home Health Care Medicare Conditions of Participation, Pennsylvania Home Health Care licensure laws and agency policies and procedures. Provides the initial and comprehensive nursing assessment of patient needs and collaborates to develop the patient centered, home health plan of care. Implements the plan of care and reevaluates and revises the plan of care relative to the patient''s nursing needs to provide appropriate, comprehensive and responsive goals and interventions. Works closely with the assigned case manager and other members of the health care delivery team to ensure all patient and family care needs are being met. Provides supervision to Licensed Practical Nurses (LPN) and HomeHealth Aides. Coordinates the weekend schedule, assigning visits to meet the home health patient needs. Essential Duties and Responsibilities include the following. (Other duties may be assigned). Performs initial and comprehensive assessment visit to develop the Plan of Care that includes realistic and measurable goals in collaboration with the patient/ caregiver and members of the health care team. Implements and evaluates the interventions identified in the patient centered home health plan of care and initiates necessary revisions. Directs and supervises home health aides, LPN, patient/ caregiver and families in the provision of care. Establishes the home health aide care plan and supervises its implementation per agency policy and procedure. Promotes continuity of care through coordination of patient services. Participates in patient care conferences with Clinical Manager/ designee and other healthcare members as necessitated by patient care needs. Documents the delivery of care to comply with regulatory requirements, agency policy and procedures, and standards of practice. Performs, cognitive, cultural, readiness, literacy, and skills assessment of patient to develop educational plan in collaboration with patient/ caregiver and members of the health care team. Implements and evaluates individualized patient education plan based on age-specific criteria. Serves as a liaison with customers to assure effective communication. Acts as a patient advocate to promote goal attainment. Maintains time management and organizational skills to achieve established productivity expectations. Adheres to all agency policies and procedures. Participates in orientation of staff. Participates in mandatory in-services and education activities as required. Assumes responsibility for ensuring own professional growth. Accountable for utilization and practice of systems thinking, teamwork and customer service. Requirements: Education Graduate of an accredited registered nursing program. Bachelor''s degree in Nursing preferred. Experience Has minimum of one (1) year general nursing preferred with current home health, medical, surgical or critical care experience. Ability to work within an interdisciplinary setting. Ability to work independently. Licensure, Registration, and Certification Registered Nurse licensed in the Commonwealth of Pennsylvania. Valid driver''s license and reliable transportation that is insured in accordance with Pennsylvania state requirements and agency policy. Satisfactory references from employers and/or professional peers. Satisfactory criminal background check. Maintains current CPR certification. Access to reliable internet access.
