DescriptionWho we are looking for: Are you the Community Health Worker we have been looking for? In this position you will be able to convey your passion for helping others by working with people in the community! JRI''s PACT program provides intensive, home-based care to people living with HIV who are having challenges navigating their healthcare and treatment. Staff assist clients in coordinating and getting to medical appointments, managing medications, and dealing with barriers to better health. PACT also provides training and technical assistance to other organizations interested in developing a community health worker program. JRI provides the training so that: YOU will serve as a medical and social advocate for youth of color, ages 13-29, who are at risk for HIV and substance misuse prevention, with a focus on those who identify as LGBTQ, people who inject drugs, experiencing mental illness, homelessness, and/or sexual exploitation. You will establish a therapeutic and trusting relationship with each client and assist the client as they connect to care and treatment, follow the treatment recommendations of their doctors, practice healthy behaviors, and obtain necessary support services. YOU will help the program improve its understanding of substance use disorder/HIV prevention needs, improve linkage to care and services for the priority population, and assist with the development and maintenance of collaborative relationships with local substance use treatment centers, AIDS service organizations, health clinics and social service providers. YOU will provide adherence-focused education, skills coaching, outreach, and accompaniment to individuals living with HIV, HCV, and substance use disorders. YOU will connect with individuals engaged in high-risk substance use behaviors through culturally competent outreach and provide them with education, accompaniment, and other resources. YOU will increase access to and utilization of culturally-relevant and respectful health care and social services for those it serves. Why JRI? Be who you are! Requirements: Candidates should have a Bachelor''s degree or equivalent and relevant professional experience. Candidates should have 1-4 years of case management/ Community Health Worker Experience. Candidates should have experience with people living with HIV, mental health disorders, and/or substance users. English (verbal and written) fluency required. Spanish, Haitian Creole, Portuguese and/or Cape Verdean Creole fluency strongly desired. Candidates should have demonstrated respect for people living with HIV/substance use disorders and their diverse lifestyles and choices. Candidates should be creative, flexible, exercise sound judgment, and be able to take initiative. Candidates should have strong communication and writing skills as well as excellent time management and organizational skills. Candidates should have excellent interpersonal skills and demonstrated ability to interact professionally with culturally and educationally diverse staff and clients. Candidates should have the demonstrated ability to work as an effective team member in a complex and fast-paced environment. An active driver''s license and access to a vehicle is required.
