The Patient Care Coordinator is responsible for the work flow coordination within Home Care Home Base. In addition, PCC must be able to do an assessment and evaluation of patients. Position Requirements Minimum of one year experience in providing medical, oncology, home health care, or hospice nursing preferred, may be specified by state. Previous management or supervisory experience preferred.
