Overview **Glendale Location** The Registered Nurse will ensure that appropriate, timely and quality clinical services are delivered in an Integrated Care Clinic to individuals and families according to JFCS policies/procedures and protocols. The Registered Nurse will assess, plan, implement, evaluate and document nursing care of clients and families, whether the method is via direct medical services or case management services. Responsibilities Performs assessments on children, adolescents and adults, with duties to include: taking height, weight, blood pressure, and BMI; ascertain if the client is taking OTC meds, PCP prescribed meds or psychiatric prescribed meds and document reported medications; assess client for allergies, adverse effects to current medications, compliance with taking meds as prescribed and complete AIMS; obtain the date of the clients last PCP visit, update PCP and school information including: (name, address, phone number, fax, and for children: their grade level and teachers name); assess if children are up to date on their immunizations; obtain, initial, date and review lab results; follow-up on abnormal lab values: staff with prescriber and notify the PCP; assist prescriber in checking to verify informed med consent is in clients chart or obtain an informed med consent, and provide medication education: written and verbal to clients and families with documentation of their understanding of the medication education given. Takes verbal and written orders by prescriber to include e-prescribing, calling, or faxing medication as indicated, lab, and EKG. Complete follow up med check/wellness calls to clients/caregivers to establish medical stability and consult with prescriber to inform of status. Participates in Clinical Team meetings as needed and med check visits when requested by therapist or prescriber. Completes prior auth renewals on medication after the initial one has been done by the prescriber; (RN and Med Coordinators do initial and all prior authorizations). Maintains the medication sample cabinet: contact drug reps for samples, maintains sample medications in an organized and labeled manner, ensures destruction of outdated meds, place expiration date on all medications in cabinet, and maintains medication records as set forth in the JFCS Policy and Procedures Manual. Supervises Medical Coordinators to include training new employees, ongoing training and weekly Supervision, and scheduling. Signs off on Medical Coordinators time cards. Clinic Super User for Medical Staff. Effectively triages prescriber phone calls and walk-in clients; staff urgent or crisis calls with the prescriber; schedules DTO/DTS clients, posts hospital discharge clients and possible psych hospital admission clients into prescribers schedule for emergent psych evaluations and emergent medication check appointments. Sees post hospital clients for assessment and evaluation and consults with prescriber for continued hospital medication management and recommendations in the event prescribers schedule cannot accommodate clients within the time frame required. Practices and maintains effective communication with site Prescribers, Clinical Team, Medical Director, the multi-disciplinary treatment team, and the site Director as necessary; provides collaboration to coordinate medical services and maintains effective communication with the clients, families, Child Protective Services, PCP, Laboratory, Pharmacies, Maricopa County RBHA, Probation and Parole Departments, Division of Developmental Disabilities, Schools, Case Managers, Group Homes, Primary Care Physician, and other providers. Works effectively within the Arizona Recovery Model as outlined by the 12 principles including: Accessible services, Best practices, Collaboration with families, Collaboration with others, Connection to natural supports, Functional outcomes, Independence, Most appropriate setting, Respect for the child and/or family''s unique cultural heritage, Services tailored to the family, Stability, and Timeliness, and complies with Abuse reporting procedures. Acts in a manner that is in accordance with current ethical standards and promotes a positive public image for the organization and the profession. All actions will be in accordance with applicable state and federal laws and the established professional standards. Works within scope of practice as set forth by Arizona State Nursing Board. Maintains medical records as set forth by JFCS policy. Qualifications Bachelors degree preferred from an accredited school of Nursing. Current and maintained Arizona State Registered Nursing license and CPR certification. Must be eligible to obtain and maintain a current DPS fingerprint clearance card. Minimum of two (2) years experience in a behavioral health setting or demonstrated equivalent experience. Experience with integration of Medical Suite and Behavioral Health.Experience providing care/injections with SMI members.
