St Xavier Home Cares Services is seeking licensed HHAs and CNAs Cases in all five boroughs, Live In, Night Shift and Day Shift. FOR TBI & NHTD PATIENTS. Walk-In Interviews welcome- 68-12 Yellowstone Blvd Unit AA Forest Hill, NY 11375 (Please bring ID, License and Full Medical if you had it done within the year) Full Time Hours Job Duties: -Assist with patients personal care needs: personal hygiene , bathing, dressing/undressing, and ambulation together with any household task (e.g making bed, changing linens, laundry, light housekeeping, etc.) essential to the patients health in accordance with the patient''s individualized care plan. -Plan/prepare nutritious meals and assist in feeding patients as directed.-Document services provided in an accurate and timely manner in accordance with company policy and personal care plan. Maintain patient confidentiality at all times. -Report any changes in the patient''s condition or incidents to RN Supervisor or therapist in a timely manner. -Carry out other duties that may be assigned -Accompany patients to their doctor visits, carry charts, take notes, and push wheelchair. Job Type: Full-time Pay: $17.00 (PCA) - $18.00(HHA) per hour
