Our services include nursing, physical, occupational and speech therapy, social workers and home health aides. Specialized programming is available for those with COPD, pain, and congestive heart failure.Job SummaryThe Physical Therapist is responsible for the evaluation, planning, directing and administering of physical therapy modalities of treatment as prescribed by a licensed physician; administers treatments and physical agents as prescribed by a referring physician in an effort to restore function and prevent disability following injury, disease or physical disability; and, assists patients to reach their maximum performance and level of functioning, while learning to live within the limits of their capabilities.Candidate ProfileCurrent PT license through the AZ State Board of Physical Therapy required.A minimum of 2 years PT experience required.Valid driver''s license with proof of insurance in accordance with state requirements. 100% travel required.COVID-19 vaccination required, or reasonable accommodation based on disability, medical condition, or religious belief that prevents vaccination.Legal DisclaimerUpon date of hire, applicants will be required to provide documentation confirming authorization to work in the United States.
