Position Summary: The recreation therapist (RT) evaluates and treats patients, maintains patient activity programs, communicates with patients, families, surrogates, conservators, physicians, and other health team members and maintains documentation of services, treatment plans etc., in the medical record. The RT will take caseload of patients for a group activity, one to one activity is appropriate, or other means of supporting the patients recovery on the unit. Essential Duties and ResponsibilitiesProvides effective group activity schedule and ensures active participation of patients as appropriate. Create all recreation therapy assessments and treatment plans for patients, completed in timely fashion in compliance with State and Federal requirements. Communicates with interdisciplinary (IDT) all clinical concerns regarding patient care. Assures that documentation is completed in a timely and accurately.Manages case load in an efficient manner. Participates in development, review and implementation of departmental policies and procedures as needed and at least annually. Participates in annual skills review as required by the hospital. Participates in hiring process, orientation, mentoring, counseling, performance evaluations and performance improvement plans for activity staff. Completes performance evaluations in a timely manner. Supervises student/interns. Participates in the annual budget process for recreation therapy services. Determines equipment andsupply needs and is responsible for purchase and inventory of supplies. Insures safety of program area and supplies / equipment. Clinical Responsibilities: Assessment / Reassessment: Assures that an initial activity assessment is completed within 72 hours of admission of patients. Assures that an activity reassessment is completed with a change in behaviors, change in condition and at least weekly. Treatment Planning: Assures that treatment plans include activity interventions on admission and activity treatment plans are completed within 72 hours. Treatment plans are updated on change of condition or behavior with new interventions and at least weekly. Interdisciplinary Team (IDT): Actively participates in the IDT process and is prepared for presentation of pertinent information during IDT including revision of treatment plans, behavioral interventions etc. Communication: Assures that communication between activity staff and the interdisciplinary team is completed in a timely manner. Attends and participates in treatment team meetings. Presents patient status (i.e. changes in behavior, responses to interventions, new behaviors and new or re-enforced interventions). Notifies the IDT members promptly of any changes identified in patient''s condition Provides information about patients to IDT members verbally and in writing utilizing the Acute Psychiatric Log as needed. Reviews Acute Psychiatric log book on a daily basis. Documentation: Maintains accurate documentation including but not limited to: Observes and monitors patients for side effects of medications, antecedents to inappropriate behaviors, changes in cognition / physical status etc. and reports to licensed nurse and / or physician for further review and possible change in treatment etc. Admission assessments Reassessments at least weekly Individual and group participation Treatment plan. Activity Groups: Provides group and one on one visit on a pre-scheduled and on an as needed basis including but not limited to: Maintains a daily / monthly activity calendar. Participates and / or leads group activities including but not limited to: ADL / grooming, aroma therapy, music therapy, art therapy, process groups, psycho-education, recreational, social, spiritual, educational, leisure skills etc. Provides supervision of patients during activities to prevent accidents i.e. falls, inappropriate eating, choking, patient to patient incidents, self-injurious behaviors, other inappropriate / dangerousbehaviors etc. Accounts for all sharps, equipment and supplies at the end of each group. Secures all materials in a locked area at the end of each group session. Patient Teaching: Instructs patients / patient''s authorized representative / conservator regarding treatment plans including leisure activities. Notification: Notifies supervisor of new behavior problems in a timely manner. Reports incidents of elder abuse per facility policy. Removes any defective equipment from immediate use using ''lock out - tag out'' procedure and notifies appropriate department / vendor for repair. Notifies Housekeeping of any spills etc. in a timely manner. Completes a facility incident report when unusual occurrences happen, participates in the investigation and notification to the appropriate facility leader. Continuous Quality Improvement Responsibilities Quality Assurance and Performance Improvement: Actively participates in the department QA & PI program. Makes any necessary corrections and makes recommendations for improvement to the QA and PI Committee at least quarterly. Theft and Loss: Coordinates the theft and loss program. Maintains a theft and loss log, tracks all incidents of theft/loss, investigates lost property, reports stolen items valued at $100 or greater to law enforcement, facilitates replacement of lost property in collaboration with Administration. Presents a report to the facility QA & PI Committee. Implements remedial actions as indicated / directed. Complaint/Grievance Process: Assists with coordination of the complaint / grievance / compliment process. Logs all complaints I grievances I compliments per facility policy. Assures that resolution is appropriate to person submitting issue. Tracks, trends and reports issues to the QA& PI Committee on a regular basis. Implements remedial actions as indicated I directed. Staff training: Provides instruction and training to patient care staff (i.e. C.N.A / M.H.W., licensed nurses, activity, social services, ancillary and other staff as appropriate) regarding behavioral interventions and psycho social issues in an ongoing manner. Patient Safety: Assures safety for patients on the Acute Psychiatric unit including but not limited to: Safe environment (i.e. doors are secured, no trip hazards, no sharps, no left over food, no products with hazardous chemicals that are not secured, no objects that could be used for strangulation or self-harm etc.) Identification of all patients utilizing two forms of ID for treatments. Fall precautions Elopement precautions Housekeeping and maintenance supplies are locked or under supervision of a staff member at all times. No prohibited items are brought on unit by patients, families, visitors, and others. General Responsibilities: Follows facility policies and procedures and federal/state regulation. Participates in facility quality management program. Follows oral and written instructions accurately. Keeps supervisor informed of work priorities and problems. Responds appropriately to emergency situations and disasters. Provides a safe environment for patients, visitors and staff. Assures security of environment to prevent exit of patients intent on elopement. Implements the facility procedure for elopement if any patient leaves the facility without permission. Follows facility standards relating to customer service. Maintains confidentiality of patients and other work related issues. Assures that all patients are treated with respect and dignity and that staff are sensitive to cultural and spiritual preferences. Follows facility rules of conduct. Attends in-service related to work and completes all annual requirement as indicated by the facility. Attends outside classes to enhance role and professional standards, as appropriate. Provides timely notification of illness/absences to appropriate supervisor following departmental policy and time lines. Attends facility meetings as assigned.Follows dress code for work area. Performs other duties as assigned. Education/Experience Requirements: Graduate of an accredited college or university with a masters degree in recreation therapy is preferred. Current certification or license- CTRS in Recreation Therapy. A minimum of one-year experience working in a psychiatric hospital is preferred. License / Certification Requirements: Basic Life Support certificate (BLS) Completion of Crisis Prevention and Intervention (CPI) training within 3 months of employment.Knowledge, Skills, and Abilities: Ability to perform activity / recreation therapy and collaborate with others; Ability to interact effectively with patients, visitors, administration, physicians, and staff; Knowledge of State and Federal laws and regulations governing Mental Health.
