Clinical Transformation Specialist RN

Staff will design, implement and continually update an educational plan for diabetes self-management according to patient needs to include information on diet, insulin, medication use, exercise and recognition and control of abnormal blood glucose levels. Patient communication methods such as motivational interviewing will occur and will be trained. Opportunities for additional certifications related to diabetes could occur. Staff will function as a transitional care nurse by overseeing the care of a patient as they move from one health care facility, such as a hospital or nursing home, to another facility or their home. The transitional care nurse ensures the relocation is as smooth as possible and helps the patient remain comfortable during the change with the ultimate end goal to keep the patient from being re-admitted and having good health outcomes.Education1) Completion of an accredited registered nurse program required.Experience1) Two years experience working in a physician group practice preferred.
