SOCIAL WORKER-LBSWCase Management Full Time! Connected Healthcare is looking for a Social Worker to join an acute care hospital near Kenmare, ND. If you are a dedicated, hard-working individual this is the job for you! POSITION DETAILS:Performs effective assessments of patients and their families. Based on needs from assessments, the Social Worker arranges, coordinates, and monitors multiple services from different providers to meet patients needs. Address psychological and social needs of the patients and forms the discharge plan in collaboration with appropriate team members. Works under the Case Management Supervisor.Responsible for conducting and coordinating patient assessments, transition of care, discharge planning, affecting patient outcomes, and patient/family education. MINIMUM QUALIFICATIONS AND REQUIREMENTS: Licensed Social Worker in North Dakota. BLS certification required.EDUCATIONAL REQUIREMENTS:Bachelor''s degree in Social Work required.EXPERIENCE REQUIREMENTS: Two years of experience in area of Social Work preferred, particularly in acute care setting. SPECIAL SKILLS OR TRAINING REQUIREMENTS: Demonstrated dependability, diplomacy, and initiative.
