Thrive every day through freedom, flexibility and a fast-paced, ever-changing, never-the-same-day-twice environment!ResponsibilitiesThe Remote Visit Nurse position supports patient self-management and improves outcomes through coaching via remote visits. The Remote Visit Nurse will provide virtual care and coordination as an important part of the the patients home health care team collaborating with patient support systems, care managers, and physicians. Communicates with all members of the patient''s care team.Provide a remote visit via zoom where the interventions and clinical needs can effectively be met.Perform virtual patient assessments and collaborate with multi-disciplinary team members (PT, OT, ST, MSW, and paraprofessionals) to achieve the patient''s desired outcomes and goals.Participate in collaborative Team/Case Conferences for patients he/she is remotely following. Assists patient in navigating Zoom technology.Assists in educating clinicians, Quality Coordinators, schedulers and others on finding patients willing to do remote visits. Supports patient self-management and improved outcomes through coaching via remote visits.Candidate must be available to attend in-person orientation in the Evansville, IN branch. Must also attend all mandatory in-services, training and education, and team meetings as required for nursing/clinical staff. QualificationsActive RN LicenseOne (1+) year of clinical experience as a Registered Nurse (RN) required. Home health experience preferred, remote visit experience preferred. Must have clinical assessment experience and be autonomous with critical thinking. Must have a professional work area to perform video visits. Intuitive knowledge of technology and comfortable with smartphones, tablets, virtual reality, interactive software and artificial intelligence.Strong Communication Skills
