As a member of the Assertive Community Treatment (ACT) Team, the ACT Employment Specialist provides direct treatment, rehabilitative and supportive services in a community setting to consumers. Provides direct job development services and employment support services to assist to obtain and maintain community-based employment. Works as a team member of a clinical team to assess consumer needs and wants, participates in the development of individual service plans, acts as a consumer advocate and liaisons with families and community agencies. Provides on-call crisis intervention services, as well as weekend and holiday coverage. Flexible schedule required.MINIMUM QUALIFICATIONS: Requires anyone (1) combination of education and years of experience below:Masters degree in a behavioral health related field. Bachelors degree in behavioral health related field required. Two (2) years behavioral health work experience preferred. Masters degree in non-behavioral health related field and thirty (30) semester hours in behavioral health education and two (2) years behavioral health work experience. Bachelors degree in non-behavioral health related field and thirty (30) semester hours in behavioral health education and four (4) years behavioral health work experience. Associate degree in behavioral health related field and four (4) years behavioral health work experience. Associate degree in non-behavioral health related field and thirty (30) semester hours in behavioral health education and five (5) years behavioral health work experience. Has high school diploma or equivalent and eighteen (18) credit hours of post-high school education in a field related to behavioral health completed no more than four (4) years before the date of proposed employment and two (2) years full time behavioral health experience OR four (4) years of behavioral health work experience. Must complete training and demonstrate competence in Integrated Care skills. Must have demonstrated experience in Microsoft Office applications. Satisfactory work records as determined by thePeople 1st Successfully completes a pre-employment drug screening. Complete all on-boarding paperwork, in a timely manner and be cleared to attend New Employee Orientation (NEO), as scheduled. Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks. Ability to meet Copas driver authorized driving requirements, set forth by Copa Healths insurance company. Including AZ Drivers license upon hire, 39-month MVR, proof of insurance and vehicle registration, if applicable. Bi-Lingual, preferred.JOB SUMMARY(Including, but not limited to) Works as part of a clinical team to facilitate recovery and inspire hope for consumers while supporting their achievement of service plan goals. Provides ongoing assessment and feedback as to their progress in relation to their service plan, regardless of the consumers current location. Works with consumers Prescribing Clinician, Nurse, and other members of the clinical team to ensure the timely development and implementation of the individuals service plan (including all necessary assessments) and their crisis plan. Implement career and rehabilitation plans as directed by the team Rehabilitation Specialist and Clinical Coordinator. Provides direct job development to the consumers assigned to the team. Facilitated informational, educational, and supportive activities including Orientation into the World of Work, the effect of work and meaningful community activities on benefits programs, job seeking skills, job clubs and employment support groups. Provide ongoing monitoring and long-term support for consumers involved in meaningful activities, including employment. Responsible for transporting consumers to necessary services, as outlined in their service plan, as needed.
