The Department of Developmental Services (DDS) is seeking motivated and experienced candidates for the role of Human Services Coordinator I. The Children''s Service Coordinator (CSC) is responsible for coordinating services for DDS-eligible children and their families within the Greater Boston Area Office. Based on a mutual planning process with the family and assessment of need, the CSC assists care-givers with planning, accessing generic community based supports, understanding SPED process and the identifying and referring to internal and external supports. Elements of the role include: service coordination, case management, information and referral, data management and inter-agency linkage. Duties and Responsibilities (these duties are a general summary and not all inclusive): Assist with the assessment of risk, needs, and the prioritization of DDS services as available and appropriate, based on standardized protocol and departmental policy guidelines. Provides oversight for DDS services and collaborates extensively and effectively with outside agencies. Oversee, monitor and develop, with families and provider agencies, Department of Elementary and Secondary Education (DESE)/Department of Developmental Services (DDS) programs (Prevention Plans) for the Area. Responsible for a caseload of individuals who are between the ages of 0 and 18. Maintain accurate and complete information in the Meditech and HCSIS systems for caseload members, including completion of progress notes, and accuracy of services enrollment/ completion. Preferred Qualifications: Candidates fluent in Spanish preferred. Comfortable with personal computers as there will be data entries required in specific databases. Please Note: Driving will be part of the duties of this position. The candidate must have a valid Mass Driver''s License and reliable transportation. Schedule: This is a full-time, 37.5 hour position. Days off to include Sunday/Saturday. Substitutions: I. A Bachelor''s degree in a related field may substitute for two (2) years of the required (A) experience. Based on assignment, travel may be required. Incumbents who elect to use a motor vehicle for travel must have a current and valid motor vehicle driver''s license at a class level specific to assignment. Based on assignment as Qualified Intellectual Disabled Professionals, within the Department of Developmental Services, a Bachelor''s degree or higher in social work, psychology, sociology, counseling, counseling education, education of the physically or emotionally handicapped, education of the multiple handicapped, education of the learning disabled, human services, rehabilitation, rehabilitation counseling, nursing, recreation therapy, art therapy, dance therapy, music therapy, physical education or other a related field is required.
