Our approaches include policy initiatives, community partnerships, and other interventions that operate within a health equity framework.The Community Health Nurse (CHN) is a member of the Communicable Disease Program, and this position''s core focus is immunization for vaccine-preventable disease. Expertise is utilized for assessment, education, evaluation, and treatment of individuals and communities, with a focus on ensuring health equity for BIPOC and immigrant communities and other focus populations such as people experiencing houselessness. Diseases of focus may include COVID-19, childhood vaccine-preventable diseases, Hepatitis A, and other communicable diseases as required.This position may also provide screening/testing, for TB, HIV, STDs, COVID-19 as needed. Regular evening/weekend work will be required; initially due to ongoing COVID response, and may decrease over time. Provision of clinical services may occur in non-clinical settings, including outdoors in a range of weatherAs the Community Health Nurse you will:* Administer childhood and adult immunizations.* Participate in preparing for and conducting mobile and community-based immunization clinics indoors and outdoors in a variety of environments and weather.* Provide health education to community based organization staff, clients, and other community members, including information related to communicable disease and health inequities.* Support collaborative community partnerships.* Peer train new hires.* Participate in planning, implementing, and evaluating collaborative community testing and/or vaccination events with community and health department partners and program leadership, including training volunteer and partner vaccinator teams.* Complete Vaccine Administration Records (VARs) and other paperwork associated with immunizations accurately and efficiently.* Provide communicable disease screening, counseling, testing and referral as directed under Multnomah County Health Department (MCHD) guidelines and procedures, at community test sites and community screening events, as assigned, including TB, COVID-19, STDs, HIV.* Review charts and lab forms, enter data into lab database, provide results via phone to clients including health education, follow-up planning, and referrals to necessary services, following the medical authorization of the Medical Director.TO QUALIFY:We will consider any combination of relevant work experience, volunteering, education, and transferable skills as qualifying unless an item or section is labeled required. For details about how we typically screen applications, review our overview of the selection process page.Minimum Qualifications/Transferable Skills*:* Possession of, or ability to obtain, an appropriate and valid license to practice as a Registered Nurse issued by the State of Oregon.* One of the following:* A Bachelor''s degree in nursing from an accredited nursing program; OR* An Associate''s degree in nursing from an accredited nursing program AND two years of registered nurse level experience is required.* Possession of, or ability to obtain, an appropriate and valid driver''s license.* BLS CPR certification required* This position will include occasional shift coverage outdoors on evenings or on weekends* This is a field based position that will be supporting mobile clinics and pop-up clinics for community events* The schedule will be Tuesday-Saturday, scheduled hours to be determined with supervisor and may include evenings and weekends depending on clinic needsPreferred Qualifications/ Transferable Skills*: You do not need to have the following preferred qualifications/ transferable skills to qualify.
