An individual that provides a broad range of non-medical, supportive services, including bathing, nail and skin care, oral hygiene, toileting, safe transfers, range of motion, nutrition and fluid intake, medication administration, monitoring vital signs, participating in household activities and any other task appropriate according to the patient''s care plan.Responsibilities:Follows a client''s written Plan of Care and carries out duties as assigned by the supervisorCommunicate professionally, including making brief presentations to patient and caregiversObserve and report any safety hazards found in the client''s home or any significant observations regarding the client. Observe, report, and document patient status and the care or service furnishedAccompany clients to appointments and events outside the homeFollow basic infection control procedures and universal precautionsReport abnormal findings in client''s condition to the supervisorReport client/family complaints to the agencyMaintain client confidentiality in accordance with HIPAA requirementsAccurately follow written and oral instructionsAttend staff meetings, case conferences, team meetings and educational in-services as requiredOther duties as assignedPossess the physical abilities to conduct the following:Frequently sit, stand, walk, bend or squatOccasionally crawl, climb, or reach above shoulder level o Occasionally lift up to and over 50 lbsContinuously see, hear, speak and use both handsRequired Qualifications:At least 18 years of ageAble to read, write and follow directionsComplete and maintain current CPR certificationComplete PPD or Chest x-ray if applicableSuccessfully complete Help at Home required training, pre-service, competency evaluation, and orientationWork positively with clients, family, staff and external agenciesDemonstrate compassion toward all persons regardless of age or any physical or mental disabilityIf transporting clients, provide proof of active driver''s license and up to date auto insurancePreferred Education and Experience:High school graduate, or completion of the equivalent educationAt least 6 months of experience providing care in a similar role The above statements are intended to describe the general nature and level of work being performed by people assigned to this classification.
