The Population Health Nurse is responsible and accountable for clinical expertise for specific patient populations for individuals with chronic conditions; potentially complex medical, mental health, and psychosocial issues. Utilizes health coaching techniques to perform disease management, assessment of disease states and referral to appropriate levels of care. Focus on identification of patients appropriate for Annual Wellness Visits and execute guided processes to complete the visit. This role requires diverse job functions to meet specific contractual and program related requirements to achieve optimal clinical outcomes.
