Our physician led medical group enjoys an excellent reputation with patients and the medical community, offering primary care, urgent care and more than 30 different specialties.Job SummaryAlong with providing behavioral health treatment to patients within primary care, the Lead Behavioral Health Clinician (LBHC) serves as a mentor and trainer for new behavioral health clinicians, in conjunction with HMG BH Director and ICP BH Director. The LBHC is responsible for assisting clinicians in attaining and maintaining productivity requirements for the PCBH providers as well as reporting on those productivity requirements. The LBHC is an administrative support to the program by partnering with Directors to troubleshoot at each Integrated location, to determine the need for new Standard Work as that need arises. They are responsible for working on countermeasures to any barrier to the success of Integrated Behavioral Health. The LBHC will support the partnership of the Care Team approach to treatment and have a deep understanding of both the Fee for Service environment, as well as the Population Health model of ICP. As these two models continue to move closer together, the LBHC will support any and all initiatives toward the improved outcomes of patients in our care. CLINICAL WORK: Provide mental health treatment to patients within primary care locations. Timely and efficient documentation on patient sessions. Collaborative Care Team approach with PCP and related providers for best patient care. LEAD WORK: Train and support new clinicians to Epic documentation specifically around PCBH templates. Further, have distinct knowledge around changes to Epic and BH reporting and relay and implement these changes with BH clinicians. Assist in the hiring process for new clinicians consisting of recruitment and informational interviews for potential candidates Assist with the expansion of tele-health initiatives and site expansion process (site selection, launch meetings, education about Integrated Care to staff in huddles) Assist in strengthening clinical knowledge base of all clinicians in this model of care. Organize lectures and trainings from outside sources. Manage ongoing PCBH education and outreach efforts (Condition of the week cards, PCBH minute education, education about integrated care and de-stigmatize BH conditions and treatment. Monitor clinician schedules and productivity and support clinicians with productivity expectations. Support PCBH productivity through schedule scrubs, site-specific referrals in epic. Intervene and collaborate with all players when a clinician''s schedule is not being met, or there are excessive no-shows, or patients who don''t follow up after intake.
