(Yes/No, If Yes please explain): No.• Credentials Required (Refer to job profile): : Current Licensure as an MA in OR (CMA, NCMA), current BLS (Basic Life Support) certification• 1 Year of Epic Experience (Required or Preferred): Preferred• Experience required: See UA.COVID-19 Vaccine (Facility Guideline):• Required - No ExemptionsSubmittal Instructions• Need 4 weeks/28 days+ at minimum lead time from Booking to Start to account for Credentialing/Compliance.• Client does not accept any traveler who has worked at Client as perm staff within the last 12 months.• MUST BE FULLY VACCINATED PRIOR TO START-- The term fully vaccinated means that they are 14 days past their final shot prior to their start date.• Please send copies of certs with profiles along with interview availability to include time zone.• Correct DOB and SSN needed.• PLEASE NOTE before submitting and after offer:• Client Forms - these will need to be completed immediately following signed contract. Recruiter needs to express the severity these forms on the front end. Otherwise, this will causer a delay in start• Fingerprint cards - It is imperative upon offer - Traveler must provide the address for us to send FP Cards• Per Client request, please VERBALLY ask traveler if they have worked at Client in the past (travel or perm) and if they have, please call it out in the submission: note the dates, the facility where they worked, and whether they were travel or perm
