We are seeking a full time Nursing Supervisor to work in our Aspirus Tivoli Community in Portage, Wisconsin. This position is responsible for direct supervision of patient care and to provide support to the Director of Nursing. The Nurse Supervisor will be responsible for the comprehensive delivery of personal care services to residents on a day-to-day basis following required regulatory requirements. The Nursing Supervisor ensures that our residents are treated with respect and dignity and ensures quality care as residents healthcare needs change. The Nursing Supervisor also coordinates, trains, and supervises resident care staff in accordance with the residents needs, state regulations, and Aspirus Tivoli Community policies and procedures. Responsibilities will include: Understands SNF Federal and State regulations and assists with maintaining compliance. Participates in facility surveys and assists with preparation, including facility entrance gathering of information and resources, facility 802 matrix, census and condition reports, audits and education related to Plans of Correction. Reviews all nurse documentation with appropriate follow-up and education if documentation incomplete. Assists with updating and writing policies. Assists with annual education in-services such as Dementia training, Resident Rights/ Abuse and Neglect, and other required training and tracking to ensure all nursing staff complete timely and remain in compliance for required education and competencies. Assists with orientation process for new employees, including assignment of mentors and completion of orientation checklist / competencies. Assists with employee performance appraisals as delegated and assigned by the Director of Nursing. Assists the facility scheduler as needed in completing CNA and Licensed Nurse scheduling to ensure adequate staffing levels. Assists with adjusting staffing levels based on census. Coordinates the Resident Assessment Instrument (RAI)/ MDS process including accurate and timely completion of resident assessments in accordance with Medicare, Medicaid OBRA, and other payor program requirements, including Care Area Assessments, and care planning. Oversees the interdisciplinary care-planning process, including initial 48 hour and comprehensive care planning. Attends and coordinates care plan conference meetings. Reviews all resident fall reports and staff injury reports. Initiates and assists Director of Nursing with investigation of incidents. Participates in IDT fall committee. Ensures nursing documentation is complete, such as post-fall monitoring, infections, etc. Assists with pre-admission screens. Ensures completeness of admission and discharge paperwork/assessments of residents. Makes daily rounds and keeps well informed of residents condition and family concerns. Supervises care provided by all nursing staff, particularly RNs, LPNs, and CNAs. Assists the Director of Nursing in coordinating and providing nursing staff meetings. Assists physician on rounds and reviews medical plan of care with physician, including reconciliation of physician plan of care (PPOC) on admission and readmission. Reviews physician progress notes. Keeps resident diagnosis list current. Consults with physician regarding recommended lab tests as indicated. Reviews pharmacy recommendations and initiates appropriate follow-up with providers. Makes environmental rounds and ensures all equipment is clean and in safe working order. Is actively involved in resident / family complaints, fall follow-up and investigation, and injuries of unknown source investigations. Communicates effectively with other departments to facilitate quality resident care and a cohesive work environment. Reviews nutritional/hydration monitoring for residents and member of IDT nutrition committee. Reviews resident behaviors and ensures proper interventions are in place. Participates in the facility behavior committee and tele-psychiatry coordination of services. Makes recommendations to the Director of Nursing for needed changes/improvement and participates in implementation. Assists with QAPI data collection and analysis and attends quarterly QAPI meetings. Demonstrates knowledge of and interest in leadership concepts in general and specifically in nursing process, quality assurance, goal setting, and holistic approach to people. Demonstrates the ability to direct team and non-supervisory employees in appropriate procedures; to communicate effectively both orally and in written format; to establish and maintain effective working and public relations; and to treat patient information as confidential. Performs other duties as assigned by Director of Nursing. Performs related job duties as assigned. Willing to work flexible hours to ensure adequate supervision of all staff. Provides on-call coverage rotation for weekends and holidays as a member of nursing leadership. Education and Experience Possesses a diploma from an accredited School of Nursing, two years of geriatric/rehabilitative nursing experience, CPR certification, and an active RN license for the State of Wisconsin. Possesses the understanding and ability to comply with Federal and State rules and regulations as it applies to Nursing Home (OBRA), Wisconsin state regulations, and Nursing Practice Act. Annual competencies as required by Aspirus and/or various regulatory agencies based on entity and/or job position.
