This role also coordinates patient needs by processing and relaying information to appropriate resources and obtaining essential information from outside resources. Assists the team with the monitoring of the telehealth patients through the telehealth software system. Works closely with Clinical Supervisors and the Intake Department to coordinate coverage of admissions and evaluations, while continuing to meet current patient needs.Core Job Responsibilities. Strong working knowledge of Home Care Services (VNA/Senior Network Health (SNH)) and community resources (i.e.; MOW, Life Line, etc.).. Provides technical support and acts as a resource to the Clinical Manager and team members.. Functions as a resource to internal and external referral services and providers in the community.. Coordinates the callers informational needs and directs them to the appropriate resource.. Enters patient information into the agency Electronic Medical Record (EMR) system as appropriate (i.e. interpreter scheduling, inpatient events, etc.).. Understands and supports agency information flow to ensure documentation tracking (i.e. Advance Beneficiary Notice (ABN)) and documentation is being completed in a timely manner.. Coordinates patient supply needs and documents in the EMR.. Knowledgeable of the call center/insurance processes and enters referral information into the EMR as needed.. Knowledgeable of the scheduling processes/computer software scheduling system.. Ensuring visits scheduled to clinicians have applicable visit types, frequencies, and durations.. Interaction with clinicians to ensure schedule accuracy and communicate schedule changes.. Viewing and investigating all open visits in EMR to ensure coverage.. Assists in covering clinician schedules due to sick calls and other unexpected changes with staff.. Assists in back up coverage to each scheduler within the department.. Rotational basis for holiday and weekend coverage throughout the year as needed.. Assists with telehealth home monitoring system requiring patient interaction via phone..
