Home Health Field RN for Southeast Suburbs

Home Health Advantage is growing, and we are recruiting for a dynamic RN to visit patients in their home and coordinate care for patient and family. You are an integral part of the patients success in managing at home as you will be coordinating with referral sources (hospitals, clinics, senior facilities) and physicians. Must have outstanding clinical assessment skills, great communication skills, ability to teach patients about their medications and disease process so patients can learn self-management. Patient caseload would be southeast suburbs of Chicago. You will need to be a teacher and patient advocate so patient goals of care are exceeded! Our mission is to exceed patient/family expectations and focusing on care excellence with every interaction!!Requirements: RN IL License, CPR, recent TB testing within 6 months, reliable transportation.
