In fact, we''re the only hospital in northwest Ohio to receive this honor.Position Summary:The Stroke Program Coordinator is responsible for the coordination of care for the stroke patient from admission to discharge. The Program Coordinator serves as a resource to staff, the inter-disciplinary team and community caring for the patients within their specific area. The Program Coordinator demonstrates knowledge and skill in the care of patient with strokes. Skills in the area of critical thinking, interpersonal communication, conflict management, problem solving, autonomy and decision-making are highly desirable. The Program Coordinator would maintain QNPI data with goals of achieving clinical excellence, including obtaining certification/accreditation requirements, as applicable. Provide timely data input and data management. Assist in the development of policies and procedures, order sets and best practice guidelines to improve quality of care and program development.The Stroke Program Coordinator is responsible for the coordination of care for the stroke patient from admission to discharge. Assist in the development of policies and procedures, order sets and best practice guidelines to improve quality of care and program development.Primary Duties: Holds primary responsibility for Joint Commission Stroke Certification compliance of The Toledo Hospital based on published standards including organizing quarterly steering committee meetings and maintaining the monthly ''Get with the Guidelines'' patient data abstraction and website entry process.Coordinates patients for stroke diagnosis throughout the transfer, ED and inpatient floor admissions.Chairs the performance improvement committee (Stroke Operational Committee) reviewing disease outcomes for stroke patients.Submits The Toledo Hospital''s data to state-wide stroke database, Coverdale Registry.Develops performance measurement systems and best practice guidelines for target populations/services.Acts as a community liaison to represent the overall goals and mission of The Toledo Hospital when promoting our services to the community and community agencies.Ensures the effective and efficient daily management of stroke patients by:Ensuring problems regarding equipment, communications and support systems are resolved. Developing and implementing operational systems. Ensuring proper patient and physician flow and implementing improvements as necessary.Establishing program goals, policies, procedures and standards; advising affected employees of changes. Upholds and works with or through the Director of Nursing to ensure departmental compliance with applicable local, state, national, Joint Commission, Ohio Department of Health, Clinical Laboratory Inspection Agency (CLIA), ANA code of ethics, and hospital and departmental standards and requirements. Overseeing the quality management, Joint Commission, College of American Pathologists (CAP) and research activities for medical, nursing, technical, and administrative personnel. Supporting research and education efforts and opportunities for staff by remaining informed of current research that affects their practice. Continuously evaluating customer satisfaction and ensuring resolution of stroke patient complaints.Supports nursing clinical practice by working with or through nursing management and Director of Nursing to: ensure professional competence and performance of clinical staff through the establishment of professional standards, protocols, policies and procedures; communication of related information. Cultivate the promotion of nursing as a profession and supporting its development. Support evidence-based practice as part of clinical practice. Ensure staff competency in caring for a culturally diverse patient population.Identifies opportunities to promote programs in professional and geographic communities.Facilitates relationships with the medical staff by; coordinating communications regarding area initiatives. Soliciting medical staff input for decision making about patient care/flow, staffing and quality of services.Responsible for participation in the resolution of clinical issues in cooperation with the management teracts with the various hospitals departments and programs regarding shared initiatives.Keeps abreast of business developments and practices by participating in continuing education courses. Professional organizations and seminars; reading current literature and maintaining professional contacts in the community. Participates in hospital required educational programs such as fire safety, chemical, right to know, etc.Bachelor''s degree in nursing required, MSN preferred. Licensed RN in the State of Ohio with five years of clinical experience. Three to five years clinical experience with demonstrated progressive management experience. Experience with the operation of health care computer systems. Excellent interpersonal skills required along with experience in working with multi-disciplinary groups. Direct clinical experience in the care of stroke patients. Experience with spreadsheet and word processing programs desired.
