Position OverviewAssist the Director of Nursing in providing and directing the health care services for Assisted Living and Memory Care residents. Supervise and direct the nursing aides in cooperation with the DON/ADON. As needed, provide personal care services, assistance, support, evaluation, and supervision to residents. Coordinate services to meet the health needs of the residents and promote wellness. Assist with pre-admission assessments of potential new residents.Work with team to develop and update service plans for residents.Help coordinate the assistance plan for at-risk residents. Activate resources through home health and communicate with the physician and responsible party regarding issues that keep the resident at risk. Based on physician orders, help guide, evaluate, perform and supervise resident assistance plans including but not limited to, assessments, resident/family teaching, development, and updating of residents'' plans of assistance. Helps ensure the implementation of the resident service plan and evaluation of that plan to include updates for significant changes.Helps establish and maintain resident records according to state and federal guidelines. Communicate changes in the resident''s overall condition to the responsible party, physician, DON, Executive Director, and team members.With the support of DON, assume on-call responsibilities and address medical emergencies as they arise. Assist with laundry and daily bed making, as needed. Assist in serving meals and bussing tables at mealtimes.Ensure the security of the Memory Care Suites according to regulations.Administer medications accurately and at the appropriate time according to physician orders.Document medications administered and those not given according to regulations and policies and procedures.Administer treatments as directed on treatment sheets.Document information pertaining to changes in resident''s functional status, personal needs, appointments, etc. Complete incident reports and documentation on resident''s record.Respond to resident emergencies. Provide first-aid assistance and arrange for appropriate medical attention/follow-up.Report concerns about a resident to the Director of Nursing and/or Executive Director.Monitors vital signs and completes required documentation.Participate in direction and monitoring of medications and other tasks through R.N. delegation.Oversee resident care aide staff, community operation, and monitor resident care on shifts when a supervisor is not present.Education, Experience, and Other RequirementsRN nursing license in the stateCPR certification requiredBasic First Aid TrainingExperience in working with older adultsKnowledge, Skills, and Abilities RequiredPromote teamwork in providing services to residents. State and Federal Guidelines for Assisted LivingNursing standards of practice for older adultsHIPAA requirementsSafe food handling proceduresPreferred QualificationsOSHA-MSDS sheets and proceduresIntermediate knowledge or willingness and ability to learn the computer programs used in this community.Previous memory care experience is preferred.Passion for working with memory-impaired senior adultsIND
