OverviewSign on Bonus Eligible! $8,000Starting pay for this position is $32.05/hour; base pay offered may increase based on experience, skills and internal equity.POSITION SUMMARY: The ED RN Case Manager is responsible for identifying, assessing, planning and coordinating, implementing, monitoring and evaluating options and services across the continuum of care for individuals in assigned case load. Health care services are coordinated on behalf of the patient to promote the delivery of care at the appropriate level of care which will optimize clinical outcomes and satisfaction with services. The Case Manager assures the designation of primary responsibility among team members to each aspect of the plan, avoiding duplication and fragmentation. Assesses and intervenes to address the complex psychosocial and behavioral health issues and needs associated with hospitalizations and transition plans. Coordinates with the health care team to facilitate and expedite the best outcomes for the patient and physician caring for that patient. The Case Manager works with the patient and treatment team to help establish goals and realize outcomes as determined by current individualized care plans. Works with the Care Management Division leadership team to ensure alignment with the strategic goals and visions. QualificationsREQUIRED QUALIFICATIONS:Education: Bachelor''s Degree in Nursing must be obtained within 3 years of date of hire into role.Licenses/Certifications: RN-Registered NurseOther Licenses/Certifications: CPR Certification within 90 days of hire Care Management certification for adult or pediatric, as appropriate, within 3 years of hire. Exam must be taken by 3rd anniversary.
