Job Description

Under the Chief Pharmacy Officer and Associate Chief Pharmacy Officer, Clinical Pharmacist Manager supports mission and goal achievement, organizational quality and patient-centered care in addition to participating in new projects to develop Clinical Pharmacy Services. The individual will be responsible to incorporate core organizational values of respect, integrity, compassion, honesty, and community in all activities and decisions. Under the provision of patient-centered care to enhance therapeutic outcomes through drug therapy optimization and medication education. Clinical Pharmacist Manager functions as both a clinician and manager dedicated to providing support of medication use systems and delivery of optimal medication therapy throughout Newton-Wellesley Hospital. Clinical Pharmacy Manager is responsible for overseeing the integration of clinical pharmacy services, drug utilization programs, and disease management and service line expansions working as a integral member of the Pharmacy Leadership team. The manager will work in a supervisory capacity to support decentralized pharmacy practice and medication use systems, dedicated patient service lines and patient-centered pharmaceutical care services, and other ambulatory specialty patient care areas requiring pharmacy collaboration and guidance. This individual will be responsible for the decentralized structure model as it relates to medication safety, as well as policy and procedure adherences to ensure standard of care practices are employed in addition to review of collected data. Clinical Pharmacist Manager sets and models the standard for excellence for clinical pharmacy practice and design pharmacy programs to collaboratively advance the Comprehensive Pharmacy Practice model. The following are essential job functions for Clinical Pharmacy Managers: Under the direction of the Chief Pharmacy Officer and Associate Chief Pharmacy Officer, designs and implements new pharmacy programs, quality improvement changes, and continuous quality assurance initiatives to advance the organization''s medication-use systems and promote optimal patient outcomes Under the direction of the Chief Pharmacy Officer and Associate Chief Pharmacy Officer, ensures pharmacy is an integral part of the health-care delivery system and facilitates enhancement and expansion of pharmacy services/programs Provide feedback for development of performance improvement plans and implement plans approved by Associate Chief Pharmacy Officer or Chief Pharmacy Officer as needed Makes appropriate evidence-based, patient-centered medication recommendations Implements, evaluates, and redesigns pharmaceutical care plans based on patient progress and evolving goals for medication usage, including drug dosing, monitoring (including nutritional support, pharmacokinetic monitoring, and anticoagulation therapy, etc.) Completes critical patient monitoring and reviews patient profile/chart to identify, prevent, or mitigate drug-related problems, improper drug or dose selection, subtherapeutic dosage, over dosage, adverse drug reactions, drug interactions, failure to receive drugs, untreated indications, medication use without an indication, and treatment failures and provide oversight and guidance to staff pharmacist as called uponWorks as an active member of multidisciplinary team and collaborates with healthcare providers within decentralized patient care areas and ambulatory care areas and executes practice priority on the delivery of patient-centered care Collaborates with the Pharmacy Operations Manager, to facilitate pharmaceutical care services, direct patient care programs, medication utilization systems within assigned services and care areas to ensures drug utilization activities are aligned with patient care needs while executing evidence-based medicine.Develops and oversees disease state management algorithms and pathways with subject matter expert buy in to improve direct patient care. Documents pharmacist interventions and reviews collated reports periodically from a trend analysis standpoint to identify and implement opportunities for improvement Tracks and evaluates assigned pharmacy programs for operational, quality, and financial efficiency working in conjunction pharmacy leadership subject matter experts and routinely benchmarks against local, enterprise and national platforms.Responds to voluntary internal Safety Report findings and works with co-dependencies to execute standardization of practice and prevent breakdowns or reoccurrence of untoward events. Represents and participates on department, hospital, and institutional medical staff forums (e.g., committees/meetings, hospital group/task forces, along with external programs/organizations) to raise the level of careCollaborates with Pharmacy Operations Manager to develop and oversee the day-to-day staffing schedule and pharmacist coverage of patient care service lines and care areas Conduct one-on-one sessions with direct reports on periodic basis to provide progress updates and assist with professional development working in conjunction with the Associate Chief Pharmacy Officer Complete performance evaluations for direct reports (as assigned by Associate Chief Pharmacy Officer or Chief Pharmacy Officer)Accountable for the financial performance, budgetary adherence of their areas and timekeeping maintain and oversight In collaboration with Pharmacy Operations Manager and Pharmacy Buyer, manages drug shortages relevant to the institution and works with enterprise sites to allow for product share Maintains proficiency in and actively engages in operational programs, central dispensing pharmacies, satellite dispensing pharmacies, and specialty pharmacy areas as appropriate for job assignment Effectively and efficiently utilizes pharmacy automation and supportive technology Facilitates practice advancement for pharmacist direct reports and establish skill block career ladder structure Serves as a preceptor/educator for student experiential education programs, including maintaining training and experience in pharmacy practice areas appropriate for teaching assignments Identify, design, implement, and assess the impact of a practice related project using effective project management skills to allow for practice advancement Maintains proficiency in decentralized pharmacy services and clinical pharmacy programs keeping abreast of currency of literature Establishes collaborative pharmacist-patient relationships and works in conjunction with Pharmacy leadership in regard to caregiver conflict mattersProficiently maintains patient information and efficiently interprets patients'' health care needs including patient problem lists and prioritized pharmacist interventions Collects and documents accurate patient medication histories and reconcile discrepancies versus orders Identifies high-risk patients and implements interventions to improve quality and safety Provides written documentation in the patient''s medical record when requested or directed by providers and/or senior leadership team Provides discharge medication review, reconciliation, and counseling as appropriate and collaborates with the team to execute hospital-wide programProvides evidence-based medication education to physicians, nurses, and other professionals Educates patients regarding safe and effective use of medications, possible medication side effects, and the role of prescribed medications in the overall therapeutic care planMaintains proficiency in hospital computer systems and medication ordering systems Provides accurate, safe, timely, and appropriate medication therapy based on patient age and needsEnsures timely medication dispensing and accurate medication availability working in conjunction the Pharmacy IS and Operations team to support cogent practice on patient care areas Interprets, evaluates, and accurately processes medication orders Identifies, mitigates, prevents, and documents problems with medication orders or therapeutic regimens, including medication errors or adverse drug reactions Communicates effectively and appropriately with health care providers and caregivers (physicians, nurses, etc.) and ensures continuity of pharmaceutical care between shifts and among staff Provides timely, comprehensive, and appropriate drug information to patients and health care providers Promote the use of the formulary by converting non-formulary medications to formulary agents when possible, evaluate appropriateness of non-formulary medication requests, and support procurement of non-formulary drugs when necessary. Follows established standard work parameters and maintains standard work environments Obtains and maintains ACLS certification and/or PALS and/or ENLSRelationships: Clinical Pharmacist Manager reports directly to the Associate Chief Pharmacy Officer or Chief Pharmacy Officer within the Department of Pharmacy and work collaboratively with operational staff, informatics staff, pharmacist staff, and technician leadership to support department initiatives. Clinical Pharmacist Manager has frequent interactions with physicians, nurses, other members of the healthcare team, students, and external professional groups.
